Do you own a Fwaggle puppy? 

Whether he or she is one, five or fifteen, we are interested in keeping track of your dog's progress, particularly with regard to health.  That is why we have elected to take part in the Canadian Manchester Terrier Club's Breeder's Check-Up Program.

We are pleased to provide a simple survey for you to complete that will provide us with the information we need to make informed decisions in our breeding program.  The information you provide will also be used during the next General Health Survey, which provides an overall picture of health concerns in the breed.  This type of information is invaluable to all Manchester breeders.

If you have never completed this form for your dog before, please do so now regardless of his/her age.  If you have been following the program, we request that you complete this form when your puppy or dog reaches his 1st, 5th and 15th birthdays.  If you forget, don't worry -- we'll be sending out reminders!  Be sure to keep your contact information up-to-date!
